MLA citation style help

Hi all..
I was surfing to find some template that could help in MLA citation style. Fortunately I found one. I missed the link, so am pasting the contents as under. This might be of some help to others.
Student second name and page number to be right aligned in header
<Student Name>
<Name and Section # of course>
<Instructor Name>
Insert Full Title of the Assignment
            From here you start off the assignment and each paragraph is indented by pressing a simple tab key when starting a new paragraph.
            Heading 1
            Guess what is the noticeable difference? You just noticed! Headings are also indented and underlined simultaneously. Do not have any formatting in the paper such as having any text bold or italics.
            There is no gap between the paragraphs. The most important part is the in text citations which uses Author, Page style where citations are listed by author(s) last name and page number range. If you’ve completely rephrased something you took from a source then you should cite like this (Haris, 21).
Subheading 1
Do not bold, italicize or underline the subheading.
Works Cited
Next page is the referencing page that is here known as works cited. Please note that references should be in alphabetical order with books coming first, followed by websites. Try placing as many books as you can. Notice that the references have hanging indents from the second line onwards and are not numbered.
A Book
Author’s last name, first name initial. Title of the book. Book Edition. City of Publishing: Publisher. (Date published).
A Magazine or Newspaper Article
Author’s last name, first name initial. “Article Title.” Magazine or newspaper name. (Date published): page numbers of article.
A Web Page
Organization/Website/Author Last Name, First Name Initial. “Web page title” Sponsoring organization or Home Page. Date Accessed.

collaborative effort(s)?

i've been having trouble putting something to these random lines i've written here and there. i don't know where to conjoin two or more, and i don't know where to expand on one or so that i've written. i'm at a complete loss these days.

at the same time, i have certain themes and subjects i want to write about, but i am completely drawing a blank here. i'm a dried up well.

Up until now, I'd spent all my valuable creative thoughts telling elaborate stories and writing pseudo-fictions on something I've never really understood. i've written innumerable pieces on the effervescent feelings of love and the malaise of not having it.
I must say, I have no right to have led a reader on like that.
Call me James Frey, if it makes you feel better (though his bullshit made a much greater read than mine ever will). For once in my life, I'm going to write about the aspects of love that I really, truly understand.

Contrary to prior belief, I can't say I've really experienced the sort of love that my writing would have one to believe.
Don't get me wrong, everything I've ever written was straight from the heart. Everything I've ever written until this very moment was a true story wearing costume jewelry.
Now i'm stripping it down.

I don't really know much about love, but one thing I do know for sure is, the only thing about being in love once that makes you want to be in love again is any happiness you felt the first time.
I can't say that I want to be in love again.
Anyone who says they want to be in a relationship has, at some point, been in a relationship where they felt happy, loved and secure.
I can't say that I want to be in another relationship either.

Another thing that I've come to terms with in love is that, with love, we're all wild roses.
You may think you're the only rose with thorns, but from a lot of listening, I've learned that every rose has its thorn.
The thing about having thorns is, when someone is willing to grab hold of you, you just have to appreciate their willingness to bleed for you.
Especially when they appreciate your ability to make them bleed.

Those are the only things I know for sure about love. Everything else is just some story I write to entertain others with my lack of comprehension of real love. Every stupid story, every stupid poem, it's all my real life, to the beat of a much better sounding drum. This though, what I've written right here, this is golden. Never forget that.
