eeerrrrrmmmmmm.....well, there are those inflatable dolls...so ppl do use dolls for non-tznius things...so perh. it is an issue? But Barbie has underwear moulded into her skin - sort of built-in gezera.

If you have big loose sleeves you can pull them down over the tefillin.
