now, if Dr. House actually taught at med school...
Because the popularity of House has suddenly exploded and there's been a sudden emergence of new fans, I figured it was high time for a House Frequently Asked Questions. Much like TelevisionWithOutPity, which has a small summary and FAQ to read before diving into the forums. And in the spirit of Dr. House, information is power. Granted, this may not stem the ever massing flow of fandom wank, but it's nice to know that not every fandom eats their own. We just happen to be snarkier than most.

1. So, I just watched my first episode of House and OMG am totally a new fangirl SQUEE yay!
Good for you. Welcome to the LJ House fandom. Watch for falling snark. The show comes on Fox at 9/8 CST, Tuesdays. The first episode was November 16, 2004, and it has been renewed for a second third fourth season, as it is constantly doing well in its timeslot. Check out TV Tome for more information about cast and such, but do be aware of spoilers.

We don't quite know why House has become so popular. We know it's a fantastic show and everything, but some of the fangirling is a bit... odd for a medical drama where the main character is a weird old guy with a cane.

2. Why is Dr. House such a gimp?
Dr. House had an infarction in his thigh, and the muscle is dead. so what is an infarction? Well, a myocardial infarction is a heart attack. If it occurs in the lungs it is a pulmonary embolism, and if it's in the brain, it's called a stroke. It basically when a clot blocks the flow of blood to muscle and tissues, resulting in lack of oxygen and consequently muscle death. There are a number of things that can cause this: deep vein thrombosis, trauma, standing for long periods of time (maybe House was a surgeon?), poor circulation, smoking. One LJ user came up with a diabetic condition! Hell, there's a higher incidence of blood clots when you are on birth control pills. The point is that the only symptom was pain, and the doctors did not treat it in time. It happened about 5 years ago, although the exact date is a little… unclear, due to a slight lack of continuity. Now Dr. House is addicted to Vicodin, a hardcore, extremely addictive prescription pain reliever. He says it "makes him neutral" so that he can do his job.

3. What kind of doctor is House?
He's a board certified head of diagnostic medicine, specialist in infectious disease and nephrology (the kidneys). He has tenure and three of his very own doctor fellows, known to fandom as 'ducklings' (see below). Cuddy says he's "the best damn doctor [Princeton-PlainsburoughTeaching Hospital] has". Dr. House is a brilliant diagnostician, but an arrogant bastard as well. A few words to describe him: closed off, curious, nosy, snarky, restless, easily bored, evasive, fickle, irritable, manipulative, perceptive, witty, adaptable, and extremely eloquent. He has a penchant for monster trucks, Nintendo D.S., jazz, rock concert t-shirts, cotton candy, and sneering at patients. He's a television fan too, liking "General Hospital" and "The O.C." He plays a mean piano. His mantra is "Everyone lies", and, unlike Hugh Laurie, has a horrible fake British accent.

4. Who are these pretty doctors and why does everyone keep calling them ducklings?
Those ducklings are House's personal diagnostic team. They are fully-fledged doctors who have passed their residency and are legally practicing doctors.

- Dr. Robert Chase: an intensivist – one who specialises in intensive care, a rare breed. Australian. House hired him because his father (a famous rhuematologist) made a phone call – saying what, nobody knows. Chase has spectacular daddy issues which are seen a lot in Season 2. His mother died from alcoholism related disease. He went to seminary school, dropped out, and has a dislike of nuns. Enjoys crossword puzzles but hates fat people. At the time of the pilot, he's been with House for a year.

- Dr. Allison Cameron: immunologist. Supposedly hired by House because she was pretty, and he was intrigued by the fact she went into medicine rather than an 'easier' career. The most emotional of the team, she was married in college to a terminally ill husband, and has a rather disturbing crush on House. Pro-life. In the pilot, she's been with House for six months and checks his mail.

- Dr. Eric Foreman: neurologist. He was supposedly hired by House because he has a (juvenile) rap sheet, after breaking into someone's house (something which all the ducklings do on a regular basis). Called the token black guy. Extremely arrogant, but not in the cuddly Dr. House way. He likes and respects House. He does whine however, and tries to listen to logic more often than House. In the pilot, he's new.

The term "ducklings" was apparently first seen by DrEpi over at TelevisionWithoutPity. It stems from a nickname at hospitals about med students/interns/residents and how they follow blindly anyone in a white coat. Sometimes they waddle after the senior doctors too. It may be from doc'lings, and is usually a putdown, but we love these ducklings.

Now, ever since TWoP started recapping House, their term for the ducklings is cottages - little Houses. The consensus is that the recaps over there aren't very good, although they are very snarky. However, their board system is highly organized and top rated.

5. That Dr. Wilson is HAWT, but why is he always around?
We don't know why Wilson is always around – except, perhaps for his undying love of House? Dr. Wilson is an oncologist (head of dept, actually), Jewish, on his third wife, wears ugly green ties and French shoes, and hits on women constantly. He's quite the charmer. Also wears a pocket protector in his lab coat, bless, but otherwise looks sharp. He spent the first season Christmas with House, and was the brains behind "Detox". House's only friend, he counts House's friendship and his job as the most important things in his life.

6. That Cuddy woman sure is mean to House! OMG they TOTALLY are my OTP! Tehehe, she has nice boobs.
Yes, that she does, and she constantly wears suits that show off her cleavage (hey, if you got it...). Cuddy is the chief of medicine at PPTH, making her a doctor, but one that only occasionally practices. She does practice her voice by yelling at Dr. House a lot though. Just for your information though, the prevailing OTP around here is House/Wilson. More obvious. We have yet to wank about ships, so it's not too big of a problem around here. Other popular 'het' ships include House/Cameron and House/Cuddy.

The canon relationships are House/Vicodin, and House/Stacy, although when she arrives at the end of season one, they have been apart for five years. We know she is a constitutional lawyer, and is still friends with Wilson.

[info]acrimonius: The real story behind Stacy and House is that House had his infarction while they were dating. Instead of amputating the leg after he was finally diagnosed and had the clot removed, House opted to go into a chemical coma for two days to hopefully ride out the toxins his strangled leg was releasing into his system; House's theory was that the damaged muscle might revive enough to allow him to have almost full use of his leg. Stacy, being House's girlfriend and House's medical proxy, overrode House's decision the moment the concoction put him in a coma. The dead thigh muscle was removed, and House woke up to being crippled and in pain for the rest of his life. Up until that point, Stacy and House had been madly in love.

Really, it's surprising what removing a little leg muscle will do to a relationship, isn't it?

7. Okay, what's all this otp, n00b, Mary-Sue thing about? I don't know what it meaaaaaans.
The Fanfiction Glossary. OTPs are good as long as there are no kerfuffles, squeeing n00bs can get annoying, and Mary-Sues are to be avoided at all cost. Slash is all but unavoidable here.

11. What's with this unaired pilot business?
A s00per-speshul Entertainment Weekly went out (to a lot California subscribers, damn them) that had a pilot episode for House. In a TV Guide too. It's basically the pilot that aired in November, aside from a few scenes. Wilson grabs House's cane, that's about it. You can find this DVD on eBay (or visit the locked LJ community to download it).

12. I have a crush on every boy! I'm going to download every episode!
Then do it at the appropriate but LOCKED LJ community - that's what it's there for. Or try a torrent - mininova perhaps? Episodes are not posted here. Please don't ask outside of a locked post.

So what is snark?
If you have to ask, you'll never know.

Finally, there are some hospital brats (those with doctors in their family, and students) in the community, but are there any actual doctors/nurses/allied health professionals around here? I'm always interested in other's views.

Diabetic condition - I mentioned earlier that an infarction in the thigh muscle is a rare complication associated with uncontrolled diabetes. I don't actually believe that will be the backstory for House (it doesn't fit), I was just pointing out that it's the most common result if you search medical case studies for thigh infarction on Google.

BTW, it's Stacey (Stacie? Stacy? Staci? - egads, no!) not Shelly who's the lost love.

The unaired pilot specifies that Wilson is currently on his third wife (House echoes the 'You love everyone. That's your pathology.' line from Fidelity before citing Wilson's three wives as an example).

FWIW I'm diabetic and had two muscle infarctions about 6 months ago.

AFAIK nobody figured out why, exactly. as of a few months ago when I did web searches on the subject, I found that there aren't really that many recorded cases, and I get the impression that most of the patients are in far worse shape than I am.

I'm on the kidney-pancreas transplant list and have been in the hospital (4 different ones) 14 times since mid-April last year, for a whole variety of things... I see docs several times a week, sometimes. I guess this makes me a hospital brat (can you be a brat at age 36?)

Due to all of the above, the show is SO TIMELY for me personally! Many of the details are wrong, but they got the feel of the whole medical thing right. Very right.

The rest of this is long and full of posibly boring details about my muscle infarction and how it might relate to House's pain and Vicodin addiction:


As far as the infarction goes, my own experience with it is this:

Limping and such: There was enough muscle atrophy to affect my ability to walk, considerably, to the point where I was even in a wheelchair very briefy. There was already a bit of diabetic neuropathy and some edema caused by failing kidneys, so it's not totally clear right now how much damage the infarction will have done in the long term.

I know that with some effort (treadmill, other exercises) I've been able to regain much of the function I lost in that leg and will probably see a physical therapist in the near future. I am still somewhat unsteady on my feet and have a noticeable limp... Hugh Laurie's simulated limp is pretty similar, actually.

Pain: Seems not to be an issue, currently. The infarctions themselves were the most painful things I have ever experienced, period. I have never previously been a frequent user of pain medication... as in, I rarely even bothered to take aspirin... would throw away the Tylenol 3 after a surgery... etc.

THIS was the kind of pain that turns a human being back into a mindless animal. Vicodin didn't even do much... oxycodone and oxycontin helped a bit, but nauseated me so much that I stopped eating. Duragesic patches did nothing except lower my blood pressure so that I passed out. The morphine and dilaudid IVs I got as an inpatient did help a little, but also caused me to stop breathing at amazingly low doses... on three different occasions. There were problems with the epidural catheter (both attempts) the pain clinic tried... oh yeah, and stuff like visualization, meditation, etc., which have helped me in the past with minor stuff, didn't touch this pain.

We finally settled on Dilaudid pills, which didn't remove the pain but did take a little of the edge off.

But... after a while the pain stopped. Occasionally I would pull a muscle or move funny and set off a few days of intense pain, but this happened less and less frequently. Interestingly I never got addicted to any of the pain meds, presumably because I really needed them? Or maybe because I lucked out and don't seem to have an addictive personality... hard to say.

While I was still using the oral Dilaudid pretty heavily, though, it seemed like everything... hmm, hurt more. As in, something that would ordinarily have been a 3 on the 1-10 [ain scale was now a 5 or 6. One doctor (who was briefly under the impression that I was developing a Dilaudid addiction and was pleased to be wrong about it) seemed to think that my lowered pain tolerance was actually the result of so many painkillers.

So, getting back to House... the fact that his leg still causes him so much pain in this case could actually be more about Vicodin than it is about the leg. Hard to say since I don't really know where his infarction was supposed to have been (my big one was in the left leg, quads) and maybe that would affect the duration and type of pain.

Also, I only have my own experience to go on... don't really know much about muscle infarctions in general.

Sorry to write a book here... I'm sure nobody has read this far :)

This rocks. Congratulations on a stunning piece of work.

On Foreman being the newbie- did you ever see the Star Magazine article scan that someone posted on the doctors being hot? Those stupid bastards called Chase the newbie. Chase AKA the one who's been there the longest. I was offended on Chase's behalf. It was so insignificant and trifling but I was nevertheless offended.

I almost feel like getting a journalfen account JUST to join the fandom_wank community. But that'd be silly, wouldn't it?

P.S. There's a real doctor on the official house forums and on the mailing lists called Mac who is often badadvi-jokingly referred to as "The House Doctor" and he's great with giving medical info to fanfic writers/anyone interested related to House, but I'm not sure if he's on lj.

Thumbs up in general. It's a very good jumping-off point for n00bs.

Howeverrrrrrrrrrr, I'm not too sure about the spoilers! Some of them are semi-necessary, such as the length of time each Duckling has worked for House, but the stuff about Chase's mom and Cam's marriage and House's possible past love? Wouldn't they be better off left unsaid so the kiddies can find out? :X

I think it's a great idea to have a FAQ. Nice going.

For what it's worth, my preference is always for capitalizing the first word in each sentence. YMMV. I probably wouldn't choose to read it in this format. If you want this to be widely read, I think that's something you should consider.

Well, I is one. I'm an RN/BSN, specializing at the present in critical care (I work in an ICU), with a background in home health care and general medicine. I'm more than willing to answer questions re medical stuff for purposes of fic or just in general.

what kind of ICU then? I like volunteering (for PT) there because it's the busiest. but as a nurse, whoo-ee, it must be a labor of love.

A s00per-speshul Entertainment Weekly went out (to California subscibers, damn them) that had a pilot episode for House.

a bit of clarification: it didn't just go out to california subscribers, it went out to a sample all over the country. it was also included in subscription *and* newstand issues of "tv guide". i don't have the dates handy, but you might look them up and include them (i know julia roberts was on the cover of EW and shania twain was on the cover of TVG) in case people want to try deeper ebay searches.

so it did go out to more states? I was miffed because even though I live in the largest city in the one of the largest states, I didn't get it. Houston always gets ignored.

had no idea it was in TV Guide too! I'll have to search deeper then.

Wait, didn't they say that Chase was from England or something? I could've sworn that they'd mentioned him living in London when he was younger or something of the sort. Never a word about Australia. Now the actor is from Aussie.

London (or something that referred to England?) sticks out in my mind because I remember when I found out that the actor was Aussie, I was thinking that I'd just assumed the dude was British because of the London reference to the show, and how I didn't think twice about the accent being slightly different..

I may be wrong, but I thought they'd made a comment about him living over in the UK. Never have I heard about an Aussie reference with Chase. Least that I remember of..

nah, it was House teasing Chase that he was a Brit because he put the Queen on the money. in "Cursed" House recognises Dr. Chase senior about the faint Aussie in his accent.

he might have lived in the UK, but Chase is an Aussie.

You seem to be confused as to how the blood clot that led to House's infarction was formed. In Three Stories, while talking about the "drug addict" patient, he eventally tells the class "it was an aneurysm that clotted. . . leading to an infarction." So that's how the clot was formed. Hope that helps ;)

And great FAQ, by the way. Fun to read!
