Excuse me if this is a little vague; I am working with a .psd
1. Start out with a black 550x300 palette. Paste on a texture by
lovelamp, invert it and delete the white border.
2. Paste on the subject, mine was this one from Keria Seduction. Do whatever minor adjustments are necessary, such as contrast and noise reduction. Erase the background.
3. Duplicate the Keira layer, resize it, flip it horizontally and desaturate it. Add a black box behind it and a thick white low-opacity border.
4. Brushwork. I used two flowery brushes by
77words in white with 40% opacity, and one in white with 100% opacity.
5. Fill a new layer with a pale blue (#E0EFF2) and set it to Color Burn.
6. Use this texture by Tre on Soft Light.
7. Fill a new layer with a tan-ish color (#F9E2B2) on Multiply. Select the center brush and play around with the Hue and Saturation until it is green. Only the flowery brush should be green.
8. Duplicate the blue Color Burn layer, bring it to the top and erase the part on the texture. 
9. Add a scratchy film grain texture (mine again by
lovelamp), some text and call it done.
